Consent To Remote Communication
By requesting or joining a discovery consultation by video or phone, you consent to participating through remote communication technology selected by the practice or supported by CaelaraHealth.
The practice may switch between video, phone, or rescheduling when needed for access, connection quality, privacy, safety, or operational reasons.
Your Responsibilities
- Use a private space where others cannot overhear unless you want them involved and the practice agrees.
- Use a stable internet or phone connection and a device with working audio.
- Tell the practice if another person is present or helping you participate.
- Avoid driving, unsafe settings, public spaces, or multitasking during the consultation.
- Keep your device charged and protect notifications, screen sharing, camera view, and microphone access.
Risks And Limits
- Internet, phone, video, or audio service may fail, lag, disconnect, or reduce quality.
- Remote communication may be overheard or seen by people near you or with access to your device, account, carrier account, or network.
- The practice may not be able to verify all information, identity details, location, or safety concerns remotely.
- Remote discovery is not appropriate for emergencies, crisis needs, or clinical evaluation requiring physical examination.
Connection And Follow-Up
The practice may ask for your name, contact information, location, or other basic details to help manage the discovery consultation. If the call disconnects, the practice may try to reconnect using the contact information you provided, but reconnection is not guaranteed.
A video or phone discovery consultation does not create patient portal access or a provider-patient relationship.
